Hospice Nurse Navigator  

Frederick Health Hospice – February 2024 – Present 

• Initiate goals of care meetings with patients/families to review MOLST/advanced directives and ensure patients treatment goals are established and followed 

•Coordinates hospice consultation meetings with patients/families to educate on hospice services and initiate transition to hospice care
